Constipation shows up disproportionately in women, and Gottfried ties it to a mix of factors: the female gut is roughly ten feet longer than a man's, borderline thyroid dysfunction is frequently missed, and higher rates of perceived stress and trauma affect the gut-brain axis. Supporting the microbiome with fiber-rich vegetables helps, and for people who resist eating them, a simple workaround is blending frozen steamed broccoli or a greens powder into a smoothie a few times a week rather than relying on supplements as a first move.

Birth control: real benefits, underexplained risks

Oral contraceptives reduce incessant ovulation and, with it, ovarian cancer risk: five years of use lowers that risk by up to 50 percent. Even so, Gottfried says they are prescribed too readily for acne or painful periods without explaining the trade-offs. They deplete micronutrients such as magnesium and certain B vitamins, raise inflammatory markers such as high-sensitivity CRP, and increase sex hormone binding globulin, which lowers the free testosterone available to the body.

That drop in free testosterone shows up as lower libido, vaginal dryness, and, according to the studies she cites, a possible clitoral size reduction of up to 20 percent. Before prescribing birth control for non-reproductive reasons, alternatives such as fish oil, specialized pro-resolving mediators, or aspirin can address period pain directly.

Perimenopause and the brain connection

Perimenopause can last up to ten years and typically starts with cycles getting progressively shorter. Starting around age 40, cerebral metabolism drops by roughly 20 percent, a shift tied to declining estrogen that women with more hot flashes and night sweats experience more intensely. This finding, linked to neuroscientist Lisa Mosconi's research, directly connects perimenopause to future Alzheimer's risk, a disease that this line of research suggests begins taking shape in midlife, not old age.

That is why hot flashes and night sweats should not be dismissed as a nuisance: they are biomarkers of cardiometabolic risk, bone loss, and brain changes that deserve a real conversation about hormone therapy, not just a cooler bedroom.

A test few doctors order

If there is one study Gottfried recommends without exception starting at age 45, it is a coronary artery calcium score, a chest CT scan that can be self-ordered in many places. A score of zero points to a strong cardiovascular outlook, while an elevated score, especially in women with PCOS or other risk factors, opens a window to act years ahead of the top cause of death in women.
