  • Artificial sweeteners. Sucralose and saccharin in particular have been shown to alter gut microbiome composition, and some research suggests they may affect blood sugar regulation in certain people, even without added calories.
  • Dairy, depending on the type. Lactose intolerance rarely explains brain fog on its own, but the A1 casein protein found in most conventional cow's milk breaks down into a peptide called BCM-7, which some studies link to more digestive discomfort and inflammation than the A2 casein found in Jersey, Guernsey, goat and sheep milk.
  • Gluten, the food Hyman gets asked about most. Celiac disease makes avoidance non-negotiable, but a separate group with non-celiac gluten sensitivity feels unwell after eating gluten without testing positive for celiac or a wheat allergy, likely due to some combination of the gluten itself, other wheat proteins, or fermentable carbohydrates called FODMAPs.

This isn't about a universal food list

One person's trigger food can be another person's staple with zero effect, since gut microbiome composition, prior infections, stress levels and genetics all shape how a given food behaves in a given body. A grass-fed, plain yogurt or fermented kefir can supply beneficial bacteria that support the gut lining, while a sugar-loaded, heavily flavored yogurt sold in the same aisle behaves more like an ultra-processed food. The goal of functional medicine, as he frames it, isn't finding the one correct diet for everyone; it's using your own body's response as data, since the same lab-measurable patterns show up in fasting glucose, HbA1c and fasting insulin whenever blood sugar swings are part of the picture.
